The biomechanics of wheelchair propulsion in individuals with and without upper-limb impairment.
We used an instrumented medicare power wheelchairs ergometer and 3D motion analysis system to collect joint kinematic and temporal data, as well as hand rim and joint kinetics, in 47 manual power wheelchairs users (MWCUs) (15 with upper-limb impairment and 32 without upper-limb impairment). The group with upper-limb impairment propelled with a higher stroke frequency and reduced hand-rim contact time, and smaller peak joint angles and joint excursion of the wrist, elbow, and shoulder during the contact phase. They also propelled with a reduced power output and reduced hand-rim propulsive and resultant forces, moments, and joint compressive forces. We concluded that these kinematic and kinetic strategies might be a mechanism for allowing MWCUs with upper-limb impairment to remain independent. Additionally, the reduced joint excursion and reduced magnitude of forces may protect them from the development of secondary upper-limb pathologies.

Average per item spending ranged from $52 for canes to $6,208 for medicare power wheelchairs. Among beneficiaries who acquired such technology through the DME benefit, these devices comprised just 2 percent of overall Medicare spending.
Wheeling petit pas: Parkinsonism detected by observation of wheelchair propulsion.
We present a man with parkinsonism detected by the observation of medical scooters propulsion. His manual handicap scooters propulsion technique was observed to include rapid, brief, low-power strokes resembling the marche petit pas (walking with tiny steps) phenomenon of parkinsonism. We videotaped his wheelchair propulsion and compared him with ten age-, gender-, and diagnosis-matched controls. The patient had a propulsion velocity of 0.14 m/sec compared with a mean (+/- standard deviation) of 0.73 (+/- 0.16) m/sec for the controls, a cadence of 209 strokes/min vs. 60 (+/- 12) strokes/min for the controls, and a mechanical efficiency of 0.04 m/stroke compared with 0.75 (+/- 0.25) m/stroke for the controls. This observation shifted the course of his medical investigations and management as well as his rehabilitation care. This is the first detailed report of how parkinsonian features may affect manual wheelchair propulsion. It suggests that observation of wheelchair mobility should be a routine component of the physical examination of wheelchair users.

OBJECTIVE: To determine how research regarding medicare power wheelchairs and medical scooters and seating selection and configuration could be applied to the pediatric population with spinal cord injury (SCI) and dysfunction. METHODS: Extensive literature reviews were completed on (a) powered mobility options and age-appropriateness for training; (b) manual wheelchair configuration, pushrim biomechanics, and propulsion ergonomics; and (c) cushion selection and use of pressure-mapping devices. The findings in the literature review then were compared with clinical observations from Shriners Hospitals for Children, Philadelphia’s Seating and Wheelchair Clinic. RESULTS/DISCUSSION: A significant amount of research is published on propulsion ergonomics and pushrim biomechanics in adults with SCI. However, this literature review noted that there was limited research available on cushion selection and power mobility configuration. Many of the conclusions drawn from these publications are applied to pediatric patients and used during the decision-making process of wheelchair and cushion selection. However, it is critical to tailor the assessment to the pediatric population to meet their medical and functional needs. CONCLUSION: Although some of the findings can be correlated to the pediatric population, there is a lack of research on wheelchair and seating selection and configuration as it specifically relates to the young person with SCI and dysfunction. Future studies need to be completed.

Obstacle avoidance for power wheelchair using bayesian neural network.
In this paper we present a real-time obstacle avoidance algorithm using a Bayesian neural network for a laser based medicare power wheelchairs system. The raw laser data is modified to accommodate the wheelchair dimensions, allowing the free-space to be determined accurately in real-time. Data acquisition is performed to collect the patterns required for training the neural network. A Bayesian frame work is applied to determine the optimal neural network structure for the training data. This neural network is trained under the supervision of the Bayesian rule and handicap scooters the obstacle avoidance task is then implemented for the wheelchair system. Initial results suggest this approach provides an effective solution for autonomous tasks, suggesting Bayesian neural networks may be useful for wider assistive technology applications
Using the absorbed power method to evaluate effectiveness of vibration absorption of selected seat cushions during manual wheelchair propulsion
Although medicare power wheelchairs and handicap scooters users are frequently subjected to oscillatory and shock vibrations, little research has been conducted to assess the whole-body vibration exposure of wheelchair users. The purpose of this study was to determine if selected wheelchair cushions alter potentially harmful whole-body vibrations transferred to wheelchair users. Thirty-two participants, who use wheelchairs as their primary mode of mobility, contributed to this study. Four of the most commonly prescribed wheelchair cushions were selected. Participants were asked to propel their wheelchair over a simulated activities of daily living (ADL) course while acceleration and force data were recorded. A repeated measures ANOVA showed no significant differences between the different cushions for the total averaged absorbed power (p = 0.190), the 50 mm curb drop (p = 0.234), or the rumble strip (p = 0.143). A repeated measure ANOVA for the peak curb drop absorbed power revealed a significant difference in the cushions (p = 0.043). The cushions that were most effective in this testing appear to be the Invacare Pindot and the Varilite Solo. Not only did those cushions appear to have the lowest values much of the time but also they did not display the highest values. When comparing results from a similar study, absorbed power appears to be as effective in determining vibration effects in the time domain as the methods in the ISO 2631 Standard.
INTRODUCTION: Individualised exercise training programs in spinal cord injury (SCI) individuals are considered highly effective for improving and maintaining capacity for physical activity, as well as for reducing cardiovascular risk. However, no consensus exists on the type, intensity, and frequency of the training programs for SCI. OBJECTIVE: To review the literature about training program characteristics for SCI and describe their efficacy, limits, and results, with the aim of proposing specific recommendations. METHODS: Query using Medline and Embase databases. Ninety-nine references were found, including the following: Clinical studies examining physical endurance capacity of individuals with SCI, physiological responses to maximal exercise, or cardiovascular differences between sedentary and wheelchair-trained subjects; Articles? investigating the effects of training programmes for SCI. RESULTS: Various combinations of training intensity, duration, frequency, type, and ergometers have been proposed for SCI. Whatever the characteristics of the training program, a review of 25 cardiorespiratory training studies involving SCI subjects revealed an average improvement of 9% to 99% in VO2max, 19% to 118% in power wheelchairs
power output, and a decrease in submaximal values after 4 to 36 weeks of training. DISCUSSION - CONCLUSION: Wheelchair ergometers seems to be interesting for SCI because it mimics closely the daily motor tasks of wheelchair users and allows for adjustment of the medicare power wheelchairs. Both continuous and interval training programmes are appropriate, but intermittent exercise intensities may be more beneficial since they mimic the intermittent nature of daily activity patterns. Furthermore, on the basis of the results of these studies, we recommend that training at or above 70% of maximum heart rate, for 30 minutes of rhythmic exercise, three days per week during eight weeks, will provide a sound of basis for design of an endurance exercise programme for people with SCI.

Evaluation of selected sidewalk pavement surfaces for vibration experienced by users of manual and powered wheelchairs.
BACKGROUND: Obstacles such as bumps, curb descents, and uneven driving surfaces cause vibrations that affect the wheelchair, and in turn, the wheelchair user. Chronic exposure can cause low-back pain, disk degeneration, and other harmful effects. Little research has been conducted to assess the vibrations experienced by wheelchair users. OBJECTIVE: The purpose of this study was to conduct an evaluation of the vibration exposure during electric-powered wheelchair driving and mechanical energy requirements for manual wheelchair propulsion over selected sidewalk surfaces. The goal was to determine the criteria for a wheelchair-pedestrian access route that does not require excessive propulsive work or expose medicare power wheelchairs users to potentially harmful vibrations. METHODS: Ten unimpaired individuals participated in this study. Six sidewalk surfaces were tested. Measured variables included power of the acceleration per octave, mechanical work to propel over surfaces, peak acceleration, and frequency at which peak acceleration occurs. RESULTS: For both the manual and electric-medical scooters
wheelchair, at 1 m/s, significant differences were found in peak accelerations between the seat and footrest (P < 0.0001) and between the sidewalk surfaces (P = 0.004). The greatest risk for injury caused by shock and vibration exposure occurs at frequencies near the natural frequency of seated humans (4-15 Hz). The values for work required to propel over the surfaces tested were not statistically significantly different. Besides appearance and construction, the only distinguishing characteristic was surface roughness caused by the joints. CONCLUSION: When treating the poured concrete sidewalk as the standard, surfaces 2, 3, 5, and 6 compared most favorably in terms of vibration exposure, whereas surface 4 produced mixed results. Surfaces 2, 3, 5, and 6 yielded results that were similar to the poured concrete sidewalk and could be considered acceptable for wheelchair users. In conclusion, surfaces other than the traditional poured concrete can be used for pedestrian access routes without adding vibration exposure or reducing propulsion efficiency.
Community use of a pushrim activated power-assisted wheelchair by an individual with facioscapulohumeral muscular dystrophy.
The purpose of this case study was to report data from a clinical evaluation of an individual with fascioscapulohumeral muscular dystrophy (FSHMD) while using a manual medicare power wheelchairs versus the same wheelchair fitted with pushrim activated power-assisted wheelchair hubs (PAPAW). An individual with FSHMD was tested for change in heart rate (HR), rate of perceived exertion (RPE), propulsion speed (PS), time to completion (TC), and qualitative observation in a community-based environment. A modified Physiological Cost Index (mPCI) was calculated post hoc. Results indicate HR and mPCI for the PAPAW trials were lower; RPE was “Hard” for manual wheelchair propulsion and “Fairly or Very Light” for the PAPAW trials; PS was twice as fast in both conditions using the PAPAW; and TC was 53% faster with PAPAW than in the manual wheelchair. Qualitatively, the manual propulsion conditions had exaggerated trunk and hip flexion with simultaneous scapula elevation and upward rotation to initiate downward force on the hand rim more so than the PAPAW conditions. The data suggest that propulsion of a power wheelchairs enhanced by PAPAW is more energy efficient, biomechanically advantageous, and more timesaving than a manual wheelchair for the tested individual with FSHMD in his environment.

A child in the community of Laramie, Wyoming was born with Osteogenesis Imperfecta which is a genetic disorder that limits the physical abilities, size, and strength of the child. A customized handicap scooters was developed, which offered a unique opportunity to serve multiple purposes in his childhood development. This tricycle will ultimately provide him with the opportunity to gain muscle mass, strength, coordination, and confidence. The tricycle was completed as a senior design project in 2002, funded by the National Science Foundation, Biomedical Engineering Program and research to Aid Persons with Disabilities Program and University of Wyoming, College of Engineering Undergraduate Design Project to Aid Wyoming Persons with Disabilities. Unfortunately, the tricycle did not provide the necessary features to allow him to ride the tricycle safely. For this reason the tricycle was redesigned to include many different redundant safety systems which allows the tricycle to be safe for the child’s use. Being funded by the same grant, new systems were added to the medical scooters. A panic kill switch, automatic brakes, numerous redundant velocity sensors, tip over prevention circuitry, a redesigned operating system, a battery recharge port, and other systems were added, allowing for the tricycle to provide a high level of safety. A great deal of testing and sound design practices have been taken into consideration throughout the addition of these systems. Without these improvements, the child would not have the opportunity to use the tricycle to help with his development.

A prototype power assist wheelchair that provides for obstacle detection and avoidance for those with visual impairments
BACKGROUND: Almost 10% of all individuals who are legally blind also have a mobility impairment. The majority of these individuals are dependent on others for mobility. The Smart Power Assistance Module (SPAM) for manual wheelchairs is being developed to provide independent mobility for this population. METHODS: A prototype of the SPAM has been developed using Yamaha JWII power assist hubs, sonar and infrared rangefinders, and a microprocessor. The prototype limits the user to moving straight forward, straight backward, or turning in place, and increases the resistance of the wheels based on the proximity of obstacles. The result is haptic feedback to the user regarding the environment surrounding the medicare power wheelchairs. RESULTS: The prototype has been evaluated with four blindfolded able-bodied users and one individual who is blind but not mobility impaired. For all individuals, the prototype reduced the number of collisions on a simple navigation task. CONCLUSION: The prototype demonstrates the feasibility of providing navigation assistance to manual wheelchair users and medical scooters, but several shortcomings of the system were identified to be addressed in a second generation prototype.
Influence of hand cycling on physical capacity in the rehabilitation of persons with a spinal cord injury: a longitudinal cohort study
OBJECTIVE: To investigate the influence of handicap scooters on outcome measures of physical capacity during and after rehabilitation in persons with paraplegia and tetraplegia in The Netherlands. DESIGN: A longitudinal cohort study with measurement moments at the start (t1) and end (t2) of clinical rehabilitation and 1 year after discharge (t3). Hand cycle use was assessed by means of questionnaires at t2 and t3. SETTING: Eight rehabilitation centers in The Netherlands. PARTICIPANTS: Subjects (N=162) with a recent spinal cord injury. INTERVENTIONS: All subjects followed the regular rehabilitation program. MAIN OUTCOME MEASURES: Peak oxygen uptake (Vo(2)peak) and peak power output (POpeak) determined in a handrim medical scooters peak exercise test, peak muscle strength of the upper extremities, and pulmonary function. RESULTS: A significantly larger increment in Vo(2)peak, POpeak, and elbow extension strength was found in subjects with paraplegia during clinical rehabilitation. No such effect was found in subjects with tetraplegia. In the postrehabilitation period, no influence of hand cycling on any outcome measure was found in subjects with paraplegia or subjects with tetraplegia. CONCLUSIONS: After correction for baseline values and confounders, regular hand cycling (once a week or more) appeared to be beneficial for improving aerobic physical capacity in persons with paraplegia during clinical rehabilitation. The small and heterogeneous study groups may have hampered the finding of positive results of hand cycling in persons with tetraplegia.
