A dynamic seating intervention for wheelchair seating discomfort
OBJECTIVE: The objective of this study was to examine the effectiveness of a new user-adjustable wheelchair seating system designed to relieve discomfort for long-duration wheelchair users. DESIGN: This objective was carried out using the newly developed Tool for Assessing Wheelchair disComfort (TAWC) as the primary outcome measure. Two wheelchair users each tested two different designs and feedback from the wheelchair users regarding the first design was used to guide development of the second design. A single-subject research methodology was used, allowing long-duration (up to 2 wks per test) evaluation of the wheelchair seating systems and comparison of subject discomfort levels with those experienced during a baseline period using their own wheelchairs. The experimental medical scooters, handicap scooters seating systems employed existing automotive seating with embedded pneumatic bladders that allowed adjustment of the seat and back-support characteristics. The test wheelchair also had tilt, recline, and elevating leg rests. RESULTS: The two subjects completed limited periods of testing with the first design, both finding poor results with either stable or increased levels of discomfort. Subject feedback was used to redesign the wheelchair seat. After redesign, both subjects tested the second design and found it substantially more comfortable. CONCLUSIONS: The selected research methodology was a very positive method for a progressive wheelchair seating design and the second design provided improved comfort for both users when compared with that experienced using their own wheelchairs and the first test wheelchair. Future research of this type of user-controlled technology is recommended

Technological advances in powered wheelchairs
During the last 40 years, there have been revolutionary advances in power wheelchairs. These unique wheelchair systems and handicap scooters, designed for the physically immobile patient, have become extremely diversified, allowing the user to achieve different positions, including tilt, recline, and, more recently, passive standing. Because of this wide diversity of powered wheelchair products, there is a growing realization of the need for certification of wheeled mobility suppliers. Legislation in Tennessee (Consumer Protection Act for Wheeled Mobility) passed in 2003 will ensure that wheeled mobility suppliers must have Assistive Technology Supplier certification and maintain their continuing education credits when fitting individuals in wheelchairs for long-term use. Fifteen other legislative efforts are currently underway in general assemblies throughout the US. Manufacturers, dealers, hospitals, and legislators are working toward the ultimate goal of passing federal legislation delineating the certification process of wheeled mobility suppliers. The most recent advance in the design of powered wheelchairs is the development of passive standing positions. The beneficial effects of passive standing have been documented by comprehensive scientific studies. These benefits include reduction of seating pressure, decreased bone demineralization, increased bladder pressure, enhanced orthostatic circulatory regulation, reduction in muscular tone, decrease in upper extremity muscle stress, and enhanced functional status in general. In February 2003, Permobil, Inc., introduced the powered Permobil Chairman 2K Stander wheelchair, which can tilt, recline, and stand. Other companies are now manufacturing powered wheelchairs that can achieve a passive standing position. These wheelchairs include the Chief SR Powerchair, VERTRAN, and LifeStand Compact. Another new addition to the wheelchair industry is the iBOT, which can elevate the user to reach cupboards and climb stairs but has no passive standing capabilities. In addition, the physically immobile patient must be seated on an ERGODYNAMIC Seating System 2000, which is inflated by the alternating pressure compressor 8080. This seating system has a deep center seam between the two ischial-support chambers, which provides a recess for the coccyx. The pre-ischial crossbar compartment inflates during each cycle to prevent the pelvis from slipping forward. It is essential that the physician of the immobile patient order two ERGODYNAMIC Seating Systems 2000 because the patient must have an additional seating system in the case one leaks. Moreover, two compressors are necessary because each compressor must be serviced after 2500 hours of use. When you stop and think about what happens in life, pressure is being applied constantly in many different arenas. An example would be rush hour traffic that puts more and more pressure on the highways. As the population grows more and more pressure is added. What happens? Do we, as a society, give up and say ‘oh, well. Too Bad? It’s just going to take you that much longer to transit into work’? Your commute time, instead of being an hour and a half, is going to be 2 hours. And in 3 years from now, it’s going to be 4 hours. Or do we take the action, as a society, as a city, as a person and do something to relieve that pressure? Yes we do something that will alleviate the pressure for all of that traffic coming into the downtown core.
We as a society, as a city, step into success. We do something to alleviate that pressure. We can learn to apply that concept in our life. Instead of taking a look at the pressure as a bad thing we can use it to build something new, a bridge to our future. Do we do what Tommy Lasorda said-misuse that word ‘pressure’ and think of it as ‘oh, I’m going to fail, I’m going to fail’ and then give up? - or do we CHOOSE to do something different and step into our own success?
We all have habits. What is your ‘it’? What is the ‘it’ that stops you? When you start to feel that pressure, what do you do? Next time that happens, I want you to stop and think, ‘what am I automatically doing here’? Then decide to do something different in order to get a different result.
When I was first introduced to this concept, I did not recognize what habit I fell into until I was all the way in it. Stress eating was one of the things I used to do. As I learn to recognize where that pressure point is, what my habit is, what my ‘it’ is, I, can learn how to change it, how to react differently.
I just watched the movie ‘What the Bleep?! - Down the Rabbit Hole’. One of the concepts in the movie is that we, as human beings, are bombarded every second with 400 billion bits of information. We only process 2000, therefore we need to choose carefully which bits we pay attention too. When we succumb to the pressure and fall into a negative habit, like I used to when I was stress eating, we are selecting only a narrow negative portion of information from the 4 billion bits..
I have decided that I am going to choose a different 2,000 bits of information. It really is just a choice.. We need to recognize what our habits, and that we can change our habits. Millions of people have learned how to stop the stress eating habit. Millions of people have learned how to quit smoking. Millions and millions of people have learned how to make their life and Starscapes.com business opportunity better, how to step into success. If they can do it, I can do it. If I can do it, you can do it. It’s all just a simple choice, to make a different decision.

We need to learn to recognize when we are backing off from pressure, when we are moving into the habits that are not success habits (the ones that we need to change). We also need to recognize the habits that we have that are good, that are leading us towards success, leading us to our financial freedom. We need to be aware so that we can make changes in our negative habits and reinforce our positive habits.
We need to learn to recognize that when we wish for something, ask for something, pray for something or when we say affirmations for something, it’s not going to just happen. It’s not going to be handed to us on a silver platter. One of my affirmations is “I am building a team of dream achievers” Now, I know that that is going to take patience and perseverance and that this whole team of dream achievers are not just going to be dumped in my lap. I have to go looking for them and attract them into my life.
As I go down the personal development journey and build my home business I come up against pressure points all the time. When I think back to the pressure points in my life last year at this time versus this year - whoa! - last year’s ones seem really simple now. What happens? We are not given exactly what we want right off the bat. But we are given the opportunity to develop the skills so that we get better. On my journey, I am becoming a better team leader. I am practicing my patience and my perseverance in my own learning so that I become that better learner that attracts, using the Law of Cause and Effect, that team of dream achievers, the team of business builders.
When that pressure is applied in our lives, patience and perseverance, is what we need in order to grow through that pressure point. Once we do grow and find different habits to allow us to get past that pressure point, to allow us to develop the skills, to learn those new things that we need to learn, we come to a better place and we can handle more pressure. We have new skills. We know something different. We react in a different away. We remain patient. We practice the perseverance and can step into success.

Influence of crank rate in hand cycling.
PURPOSE: The purpose of this study was to determine gross mechanical efficiency (ME) at different power output (PO) levels of synchronous hand cycling and to evaluate the influence of increasing PO by changing crank rate or resistance in medicare power wheelchairs and power wheelchairs users with experience in hand cycling. METHODS: Nine male participants with spinal cord injury randomly performed three maximal incremental hand cycling tests using a computer controlled cycle ergometer. Each test started at a PO level of 50 W with increments of 10 W. In the velocity protocol, PO was increased via crank rate while resistance was constant (VEL). In the resistance protocol PO was increased via resistance while crank rate was constant (RES). In the freely chosen frequency protocol, the participants could freely select their crank rate while resistance was automatically adjusted to obtain the desired PO (FCF). RESULTS: Peak physiological responses were similar in all three tests, whereas PO max was lower in VEL compared with RES and FCF. Similar values for gross ME were found in both RES and FCF protocols, although systematically higher and increasing crank rates were adopted throughout FCF. Nevertheless, differences in gross ME at comparable relative (RES > VEL at 60 and 80% of PO range: 14.09 and 14.40% vs 13.02 and 13.11%, respectively) and absolute (RES, FCF > VEL at 90 W: 14.47, 14.47, and 13.43%, respectively) PO levels were demonstrated. CONCLUSION: These results suggest that during synchronous hand cycling the freely chosen crank rate is not necessarily the most economical, that high crank rates result in a lower ME at a given PO and that freely chosen crank rates increase with increasing PO levels.
Comparison of blood lactate elimination in individuals with paraplegia and able-bodied individuals during active recovery from exhaustive exercise
BACKGROUND/OBJECTIVE: The aim of the present study was to compare blood lactate elimination between individuals with paraplegia (P) and able-bodied (AB) individuals after strenuous arm exercise. METHODS: Eight P and 8 AB men (matched for age, height, and weight) participated in this study. Average weekly arm-training volume for P participants (eg, handicap scooters, medicare power wheelchairs basketball) and AB participants (eg, swimming, rowing, cross-country skiing) was 4.1 +/- 1.6 vs 2.8 +/- 0.8 h. A maximal-arm-cranking intensity-graded exercise test to volitional exhaustion was performed by all test participants. Immediately after the exercise test, the participants performed arm cranking for another 30 minutes at a workload of one third of the maximally achieved power output. During this active recovery, mixed-capillary blood samples were taken for lactate analysis. RESULTS: The lactate accumulation constant was significantly higher for P individuals, whereas the lactate elimination constant showed no significant difference between the two groups. CONCLUSIONS: Individuals with paraplegia seem to have no disadvantages in lactate elimination after exhaustive arm exercise compared with able-bodied individuals
Relationship between resultant force at the pushrim and the net shoulder joint moments during manual wheelchair propulsion in elderly persons
OBJECTIVE: To determine the relationship between the resultant force at the pushrim and the net shoulder joint moments during manual wheelchair propulsion in elderly persons. DESIGN: Convenience sample. SETTING: Motion analysis laboratory. PARTICIPANTS: Older manual wheelchair users (N=14; age, 68.2+/-5.2y) were tested. INTERVENTIONS: Kinematic and kinetic data were collected during manual wheelchair propulsion at a speed between 0.96 and 1.01m/s for 10 seconds and at a power output around 22.4W on a medicare power wheelchairs ergometer. MAIN OUTCOME MEASURES: Net shoulder joint moments were computed with an inverse dynamic model. The mechanical use of the forces at the pushrim and the mechanical fraction of effective force were measured during propulsion. RESULTS: Mechanical use and mechanical fraction of effective force had a positive and handicap scooters significant correlation with the net internal (P<.05) and external (P<.001) shoulder rotation moment, the net flexion (P<.05), and extension (P<.001) moment in the sagittal plane, and the net flexion (P<.001) moment in the horizontal plane. Power mobility driving training for seniors: a pilot study
This article describes two handicap scooters mobility training protocols used with seniors and compares posttraining driving performance. Twelve users of power mobility were consecutively recruited from two residential facilities in Toronto, Canada. The aim of training at both sites was to make clients comfortable with and safe at driving medical scooters devices. The content of training was similar, but training protocols differed significantly in terms of the number of sessions (means of 3.43 vs. 9.80; p < or = .05) and the time frame over which the sessions were offered (means of 1.57 vs. 5.10 weeks; p < or = .01). Participants at the two sites differed significantly in terms of overall driving performance (p < or = .05), gender (p < or = .01), and type of device used (p < or = .05). Overall, driving performance was significantly associated with facility, gender, type of device used, and training duration (p < or = .05). When these variables were entered into an exploratory hierarchical regression, facility accounted for 64% of the variance in driving performance. When facility was controlled for, the correlations between device and duration of training with driving performance were no longer significant. The determinants of driving performance are difficult to clearly specify as the variable facility encompasses gender as well as all other differences between the two training protocols. Nevertheless, these data provide direction for future research in this area.
Perceptions of power mobility use and safety within residential facilities.
BACKGROUND: medicare power wheelchairs enhance quality of life by enabling occupation, improving self-esteem and facilitating social interaction. Despite these benefits, the risks associated with power mobility use raise serious concerns in residential facilities. PURPOSE: As there is no gold standard to assess when a client is unsafe, a two-phase study was conducted to develop client-centred guidelines for handicap scooters use. METHOD: In the first phase of the study, presented here, 18 in-depth, qualitative interviews were conducted with a variety of stakeholders, including power mobility users, other residents, staff and family members. RESULTS: A thematic analysis of the interviews revealed four main themes: 1) the meaning of power mobility, 2) learning the rules of the road, 3) red flags: concerns about safety, and 4) solutions. PRACTICE IMPLICATIONS: Given the importance of power mobility, safety measures need to address issues of mobility and safety for power mobility drivers and those around them.
